Our Commitment to Your Privacy
The Center for Better Aging (CBA) is committed to protecting the privacy and confidentiality of all Personally Identifiable Information (PII) and Protected Health Information (PHI) we collect as part of our member navigation, outreach, and care coordination services.
Through this Notice, CBA intends to assure that PII and PHI are properly protected while allowing the flow of information needed to provide and promote high-quality healthcare. CBA respects the privacy of patients' health information and Uses and Discloses the information only as permitted or required by law, including the Health Insurance Portability and Accountability Act of 1996, as modified by the Health Information Technology for Economic and Clinical Health Act (the HITECH Act), as implemented in the HIPAA Rules (collectively with all implementing regulations, HIPAA).

Important Distinction: Clinical Care vs. Navigation Services
Clinical services provided at our partner clinic are delivered by practitioners employed or contracted by St. Bernard Hospital and are governed by their Notice of Privacy Practices.
This notice applies specifically to services provided by CBA staff, including
- Member navigation
- Outreach and engagement
- Health and social needs assessments
- Care coordination
What Information We Collect
As part of our services, we may collect and maintain information about you, including but not limited to:
- Name, address, phone number, and other contact information
- Date of birth and demographic information
- Health conditions, medications, and functional needs
- Behavioral health and wellness information
- Social needs (e.g., housing, food access, transportation)
- Insurance and eligibility information
- Notes from outreach, assessments, and care coordination activities
This information may be collected directly from you or received from authorized entities responsible for your healthcare, such as:
- Hospitals and healthcare providers
- Medicaid Managed Care Organizations (MCOs)
- State agencies
- Community partner organizations
How We Use Your Information
We use your information to support your health and well-being, for example:
- Coordinating care with your healthcare providers
- Connecting you to community resources and services
- Conducting health and social needs assessments
- Supporting program operations and quality improvement
- Communicating with you about services and support
How We Share Your Information
We may share your information when necessary to support your care and services:
- With healthcare providers involved in your care
- With Medicaid MCOs or state programs that authorize services
- With community-based organizations providing services to you
- With contractors or partners who support our operations (as permitted by law)
We only share the minimum amount of PII or PHI necessary required to perform the intended purpose of these functions.
Any sharing of PII or PHI by CBA shall be based on either valid HIPAA authorization, legal requirements, or required for HIPAA compliance. We will obtain your authorization when required by law, including but not limited to:
- Certain sensitive information
- Uses not related to your care or services
How We Protect Your Information
We take all appropriate steps to protect your information, including but not limited to:
- Limiting access to staff who need the information to do their jobs
- Using secure systems to store and manage data
- Training staff on privacy and confidentiality requirements
- Monitoring for and responding to potential privacy incidents
Your Rights
You have rights related to your information, which can include the right to:
- Request access to the information we maintain about you
- Request corrections to your information
- Request limits on how your information is shard (where applicable)
- Ask questions or file a complaint about your privacy
